$50 Mastercard Reward Card Video Only Offer: Customer must order new DIRECTV Beam service with 24-month agreement or DIRECTV Stream subscription with a minimum of one DIRECTV Stream box, through participating retailer, between 11/1/2022 – 03/31/2022. Must be new residential customer of DIRECTV. Activation of DIRECTV ENTERTAINMENT Package or above required. Upon DIRECTV service activation, customer must complete Mastercard Reward Card redemption form, attach a copy of first month’s DIRECTV bill showing activation and mail documents to the address listed above. Documents must be mailed within 60 days of activation of the DIRECTV service installation date. Upon receipt of all required documents, NMG will verify eligibility and ship one (1) Mastercard Reward Card to the name and address used to activate the DIRECTV account.
